Puentes de Salud exists to advance health equity through community collaboration and transformative learning. They work hand-in-hand with Philadelphia’s Latine immigrant communities to address immediate needs in healthcare, education, and social services, while fostering long-term prosperity and resilience. They also cultivate a responsible learning environment for future healthcare providers, educators, and artists to explore the intersections of social justice, structural violence, and health, preparing them to serve marginalized communities with humility and purpose.
Puentes de Salud was funded to sustain its integrated, culturally responsive model of care for Philadelphia’s Latine immigrant community. Funding will support trusted staff providing mental health care, case management, health navigation, food access, community outreach, and opportunities for connection and collective healing.
Puentes de Salud received a grant to to support their efforts providing integrated, culturally responsive health care to the Latine immigrant community in Philadelphia. Puentesʼ holistic approach includes a nurse-managed medical clinic, multi-generational education programs, and promatoras (community health workers), ensuring services are accessible and grounded in the community’s lived experiences. By addressing both immediate health needs and long-term well-being, Puentes de Salud empowered individuals to thrive and fully participate in their communities.
Puentes de Salud received support for The Building Community-Centered Mental Health Capacity program, which strengthened culturally responsive mental and behavioral health services for Latine immigrant communities in Philadelphia. Behavioral health consultants continued their community education and clinical work and promatoras were supported to become primary conveners and facilitators of support groups through skill-building and leadership training.

Puentes de Salud was awarded a grant to support their Mental and Behavioral Health program, which serves largely Latinx immigrants in Philadelphia. The grant allowed Puentes to add a male therapist to their staff and better to serve their male clients. Additionally, behavioral health therapists provided one-on-one sessions and developed group sessions, co-designed with the Puentes promatoras.
