This project aims to build a real-time clinical dashboard that links electronic health record data with HealthShare Exchange regional data to track patients with Opioid Use Disorder (OUD) across emergency, inpatient, and outpatient settings. By applying a "cascade-of-care" framework to high-acuity hospital environments, the tool aims to identify patients earlier in their care journey and detect specific gaps where individuals fall out of treatment. Ultimately, this data-driven approach will support both immediate clinical interventions for individuals and regional system-level improvements to enhance long-term recovery outcomes in Philadelphia.
