Providence Center will utilize the Overdose Prevention and Community Healing Fund funds to hire a bilingual case manager to work directly out of our community center. This position will promote and facilitate linkages to treatment and recovery utilizing a brokerage case management model, an evidence-based model in which case workers help a client identify their needs and refer them to existing services and resources in the community based on these needs. They will also serve as an advocate to ensure that clients receive appropriate, high-quality care. As part of our network of partners, this position will have access to an AirTable referral tool which has already been created for use within the Latino Equitable Development Collective (LEDC) and which facilitates connecting clients between organizations in order to reduce service time and securely share information across service providers. This is a need that has continually come up in our community and among our clients. Our goal is to streamline the process and improve accessibility to minimize the obstacles preventing community members from accessing opioid use and overdose prevention services.
