This project aims to build a real-time clinical dashboard that links electronic
health record data with HealthShare Exchange regional data to track patients
with Opioid Use Disorder (OUD) across emergency, inpatient, and outpatient
settings. By applying a ""cascade-of-care"" framework to high-acuity hospital
environments, the tool aims to identify patients earlier in their care journey and
detect specific gaps where individuals fall out of treatment. Ultimately, this data-
driven approach will support both immediate clinical interventions for individuals and regional system-level improvements to enhance long-term recovery outcomes in Philadelphia.

Building a Scalable & Sustainable Model for School-Based Health in Philadelphia: Our overarching project seeks to implement a scalable, sustainable model for high-quality school-based health centers (SBHC) within The School District of Philadelphia (SDP). SDP will own the model, and Jefferson will be the clinical partner that supports planning, implementation, and evaluation. Our project team will 1) work with experts in school-based integrated healthcare delivery to assess existing models of care and identify best practices; 2) convene diverse stakeholder groups to gather needed data to ensure that the proposed model addresses community needs; and 3) develop initial evaluation metrics and plans. The end goal is to then support SDP to connect and partner with health systems in Philadelphia to replicate and scale the model across many of the District’s 238 schools, with the potential of advancing access to care for over 114,000 families.

With support from the Scattergood Foundation, Jefferson's Trauma Education Network (J-TEN) expanded trauma-informed education through monthly trainings, workshops, and an annual conference that reached professionals, students, caregivers, and community members across the region and beyond. Building on this work, J-TEN partnered with the Southeastern Pennsylvania Transportation Authority (SEPTA) to develop a trauma-informed online training focused on stress, conflict resolution, and strategies for improving workplace communication and culture. The self-paced course was designed for integration into SEPTA's existing learning platform, extending trauma-informed practices to its workforce.
